FREQUENTLY ASKED QUESTIONS

 

PAYMENT, BILLING AND INVOICE, CANCELLATION POLICY

Cash or check are acceptable forms of payment.You will be expected to pay at the time of each session. 

An invoice and billing statement will be provided to you at the end of each month.

You can request an invoice any time you need one.

Occasional cancellations are accepted.   You will be billed for the session if you provide less than 24 hours notice.  This includes missed sessions.

INSURANCE

Dr. Bonafina is an out-of-network provider. Many insurance companies cover 50-80% of a provider’s fee, once your deductible is met. This means sessions may be covered partially or fully by your health insurance. The amount covered by your provider will vary based on your plan. You will be provided with a detailed invoice that you can submit to your insurance for reimbursement.
